After talking to a Rosa from Changi General Hospital, I found out that the only subsidized treatment (meaning getting a referral letter by a dental polyclinic) available, is Wisdom tooth surgery. The rest of the dental procedures would have only private rates.
Below are my findings:

The entire process consist of 3 visits:
The 1st Visit consist of a consultation and X-Rays. 2nd visit is the actual wisdom tooth surgery followed by the 3rd visit, to remove the stitches and get reviewed by the dental surgeon.
When I asked her why there was a difference for Medisave claimable percentage between private and subsidized, she could really tell me. From my understanding though, the Medisave claimable amount for wisdom tooth surgery is $800, therefore the extra $75 will have to be borne by you.
So if you stay in the East and want to go for a subsidized rate, head on down to Changi General Hospital.
